Overview
We believe that radical collaboration is required to reach the 5 billion people who lack access to health services.
That’s why we collaborated with Sanofi and the social enterprise, Reach52 to adapt and expand Medtronic LABS’ award-winning healthcare delivery models to The Philippines and Cambodia.
The two country programs combine engaging group-based care for hypertension and diabetes with medication delivery and telemedicine.
To ensure sustainability, the Medtronic LABS program is implemented through existing networks of Reach52 sales agents and community nurses. Padayon and Sakam – meaning ‘Move Forward’ – shows how our replicable healthcare models scale through innovative partnerships.

Community screening
Community health nurses and sales agents build awareness and screen community members for hypertension and diabetes leveraging point of care diagnostics.
Our health workers piggy-back on existing campaigns and initiatives in the hardest to reach rural areas.
Diagnosis and enrollment
Those eligible can enroll in the Padayon program on the spot.
Each patient is assigned to a social support-group that will form the basis of their experience.
Patient engagement
Patients then kick-start their journey with an intensive 12-weeks of coaching curriculum with their support group, either in-person or remotely.
After completion, patients continue to engage and receive at-home monitoring and coaching.
Clinical management
Throughout the program, patients have access to clinicians via telemedicine.
In addition, any prescribed medications for hypertension and diabetes are delivered directly to patients leveraging supply chain partnerships and delivery channels.
