Akoma Pa, meaning ‘healthy heart’ in Twi, exemplifies true health system transformation through a multi-stakeholder partnership.

Overview

Akoma Pa, ‘healthy or good heart’, is a partnership in its truest sense: we each bring something to the table.

We believe that the radical transformation of care delivery for hypertensive and diabetic patients requires a strong cross-sector partnership and an end-to-end model that is affordable at the point of care. To this end, Medtronic LABS brought two trusted partners together – The Christian Health Association of Ghana (CHAG) and Novartis Global Health – to form the most ambitious program for chronic disease in Ghana.

Akoma Pa combines the Medtronic LABS care model, SPICE platform, and field operations, CHAG’s clinical excellence and infrastructure, and the Novartis portfolio of high-quality, low-cost medications.

How it works

Community-based screening

We work alongside Community Health Volunteers (CHVs) to screen people directly in the community.

Promoting awareness about hypertension and diabetes plus screen patients for the same. Leveraging our SPICE technology for community screening and point of care diagnostics.

Enrollment and aftercare

After confirming diagnosis, patients are referred to the nearest health facility where medical staff enroll patients into the Akoma Pa program.

A risk score is ascribed to each patient based on the WHO-approved clinical algorithm, pon enrolment, each patient receives a risk score, based on our WHO-approved clinical algorithm. The risk score supports a personalized care plan for each patient.

Customized care plan

Patients engage in services customized to their unique care plan.

Community Health Volunteers follow-up with patients to monitor blood pressure and blood glucose. Patients inform the CHVs by participating in tailored tele-counseling, SMS-based programming, and peer support groups right from their community.

Clinical support

Care teams have access to real-time data to enable enhanced disease management.

If a patient receives a high reading during a CHW follow-up visit, the clinician is immediately notified. Clinicians can follow-up with patients via tele-medicine, prescribe medication digitally, edit the care plan, and more.
