Overview
Our work in Ghana represents true system-level transformation. The journey began with a shared commitment to develop and test a new model for technology-enabled care delivery with the Christian Hospital Association of Ghana (CHAG). Today, Medtronic LABS is pairing our data-driven, community-based care model with an integrated approach to primary care within the government and faith-based health systems. We’re also testing novel value-based care models with the National Health Insurance Association of Ghana for long-term sustainability.

Ever since we joined the program our health has improved. We now take our prescribed medicine from the doctor and anytime we check our glucose and BP levels the results put a smile on our faces.

How it Works
SPICE, our open-source technology platform, supports outcomes-focused patient care.
Community Screening
Community health workers (CHWs) screen patients for a range of conditions including hypertension, diabetes, malaria, TB and more, leveraging point-of-care diagnostics and symptom-based algorithms. Those eligible are referred to the nearest health facility.
Diagnosis and Risk Stratification
Medical staff confirm the diagnosis, and patients enroll in our programs. Upon enrollment, each patient receives a risk score, based on WHO-approved clinical protocols. The risk score supports a personalized care plan.
Community-based Management
CHWs follow up with patients directly in the community to monitor relevant conditions. The cadence of follow-ups is driven by the risk-based care plan for efficient and effective care management.
Patient Engagement
Patients engage in services customized to their unique needs.
Clinical Decision Support
Clinicians have access to longitudinal patient records and follow-up with patients based on real-time data. Throughout the program, clinicians can update the care plan, order prescriptions or labs, conduct medical consults and more.
